REGRISTRATION FORM INSTRUCTIONS - Do NOT attempt to manually send this to Regis Middle School. Open the Form - Complete it using the text boxes provided. When finished, click the SUBMIT button in the upper right corner of the form or the PRINT button in the upper right corner of the form. By choosing to use the SUBMIT button the file will automatically be
emailed to the school office.
If you Print the page you will need to hand deliver it to RMS.
